2.1 From service participants and their families: name, contact details, date of birth, NDIS or DVA participant details, plan information, support needs, clinical information necessary for care delivery, and emergency contact information.
2.2 From donors: name, email address, postal address (where provided), payment information processed by our payment partner (we do not store card details), and donation history.
2.3 From job applicants: name, contact details, resume, qualifications, work history, references and right-to-work documentation.
2.4 From website visitors generally: standard web log information (IP address, browser type, pages visited, time and date of visit) for analytics and security.
3. How We Collect I
3.1 Directly from you when you contact us, fill in a form, make a donation, apply for a role, or engage our services.
3.2 From third parties where you have authorised the sharing (for example, a Support Coordinator referring you to us, a treating GP, or My Aged Care providing your assessment).
3.3 Automatically through standard web technologies (cookies, analytics, server logs) when you use our website.
4. Why We Collect It
4.1 To provide health, disability, aged care and community services.
4.2 To communicate with you about our services, your enquiry or your donation.
4.3 To comply with our legal, regulatory and clinical record-keeping obligations.
4.4 To process and acknowledge donations and issue tax-deductible receipts.
4.5 To improve our services and our website.
4.6 For research, evaluation and quality improvement (de-identified where possible).
5. How We Use and Disclose It
5.1 We use personal information only for the purposes stated in this Policy or for purposes you have separately authorised.
5.2 We disclose personal information to: support professionals and clinicians delivering your care; funding bodies including the NDIA, DVA and Department of Health and Aged Care, where required; payment processors for donations; cloud and IT service providers under appropriate confidentiality arrangements; and law enforcement, regulators or courts where required by law.
5.3 We do not sell, rent or trade personal information.
5.4 Donor information is not shared with other charities or external mailing lists.
6. How We Store and Protect It
6.1 Personal information is stored in secure electronic systems with access restricted to authorised staff.
6.2 Health information is stored in line with health records legislation and the National Disability Insurance Scheme (Quality and Safeguards Commission) Rules.
6.3 We use industry-standard technical security measures including encryption, access controls and audit logging.
6.4 We retain personal information for as long as required to fulfil the purposes for which it was collected and to meet legal record-keeping obligations. Health records are retained in line with applicable legislation (typically a minimum of 7 years from last service, longer for minors).

7.2 You may request that we correct inaccurate or out-of-date information.
